DEAR MAYO CLINIC: I am 48 and the mother of two children, both teenagers. I have had an active sex life with my partner for more than 20 years, but recently I have been experiencing pain and increased vaginal dryness. I know I'm of the age that I might be going into menopause, but should I consult with a Doctor? Is there anything I can do other than abstain from intercourse or use additional lubrication?

ANSWER: Vaginal dryness can cause painful intercourse and persistent pelvic pain, among other symptoms. While it's more common in menopausal women, it also affects younger women, including those in perimenopause, which you may be experiencing.

A handful of factors can affect vaginal dryness. Estrogen is the most important hormonal influence on the health of the vulva and vagina. Low estrogen can contribute to dryness. Diet and the use of certain medications also are important factors. You can expect any medications that cause dry eyes and dry mouth to have a similar effect on the vagina.

Estrogen levels in the blood vary during the month and follow a common pattern to each menstrual cycle. For women who are not on hormonal birth control, levels are lowest in the days just before and after the start of menstrual bleeding. This low level sometimes can contribute to vulvar and vaginal dryness. Very low-dose hormonal birth control can contribute to dryness.

My recommendation is that women bothered by vaginal dryness should have a thorough exam and discussion with their primary health care provider or Gynecologist to determine the most likely cause for these symptoms. Other causes of vulvovaginal pain and irritation may be responsible for the symptoms.

A physical exam will ensure the correct diagnosis is made and that less common, but more serious, conditions are not missed. Your health care provider will help you determine the best course of treatment. While painful intercourse may limit your desire for sexual intercourse, you should not feel avoiding intercourse is your only course of action. In fact, regular intercourse can maintain vaginal health, and avoiding it may allow the problem to get worse.

To remedy vaginal dryness and painful intercourse in the meantime, consider using vaginal moisturisers, such as Trimo-san or Replens, applied every few days or about two to three times per week to moisturize and keep vaginal tissues healthy.

  • Avoid tight-fitting clothing and pantyhose. Wear white, 100% cotton underwear.

Personal lubricants and vaginal moisturisers are effective in relieving discomfort and pain during intercourse for women with mild to moderate vaginal dryness.  Both personal lubes and vaginal moisturisers reduce the friction associated with thin, dry genital tissue that can occur.

Personal lubricants

Astroglide is available either as water, silicone, organic oil base and are applied to the vagina and vulva (and the partner’s penis if required) prior to sex. Astroglide acts rapidly to provide short-term relief from vaginal dryness and related pain during sex. They are particularly beneficial for women whose vaginal dryness is a concern only or mainly during sex.

Astroglide water-based lubricants have the advantage of being non-staining. Excipients acting as humectants, emollients and preservatives are added to Astroglide water-based lubricants to achieve viscosity, alter water activity, and prevent bacterial contamination, and these will have an impact on pH and osmolality values.

Vaginal moisturisers

Vaginal moisturisers rehydrate dry mucosal tissue and are absorbed into the skin and adhere to the vaginal lining, thereby mimicking natural vaginal secretions. Vaginal moisturisers are intended to be used for the non-hormonal alleviation of vaginal dryness/atrophic vaginitis/vaginal atrophy and they are applied regularly, from every day to once every 2–3 days. Their frequency of use is directly proportional to the severity of atrophy (i.e. the more severe the atrophy, the more frequent the application), and their effects are more long-term than those of lubricants, lasting 2–3 days. Vaginal moisturisers provide this longer relief by changing the fluid content of the endothelium and lowering vaginal pH, thereby maintaining vaginal moisture and acidity. They are therefore particularly beneficial not only for women with  pain during sexual activity, but also for women who are not necessarily sexually active, but experience day-to-day discomfort. As vaginal moisturisers are intended to moisturise the mucosa, the majority contain water. In order for the water to adhere to the mucosa, they also contain either plant-based or similiar polymers. In addition, they contain a wide range of other excipients to provide the appropriate viscosity, pH buffering and preservation, and it is these additional ingredients that will affect the pH and osmolality of the moisturiser.

What is vaginal dryness?

Vaginal dryness is a painful symptom that affects a person’s quality of life. It can cause pain during sitting, exercising, peeing and sexual intercourse. Normally, your vaginal lining is lubricated with fluid that helps keep it thick and elastic. Vaginal dryness happens when the tissues in your vagina are dry, thin and not well-moisturized. This leads to discomfort, especially during sex.

Vaginal dryness occurs at any age. It’s most common in women or people assigned female at birth (AFAB) during or after menopause when estrogen levels decline. The hormone estrogen helps keep your vaginal lining moisturised and healthy. Low levels of estrogen cause your vaginal walls to become thin and dry. This is a common condition of menopause called vaginal atrophy.

Many safe and effective treatments are available for vaginal dryness.

Is vaginal dryness common?

Around 17% of people assigned female at birth (AFAB) age 18 to 50 report problems with vaginal dryness during sex, even before menopause takes place. Over half experience vaginal dryness after menopause.

Possible Causes

What are the possible causes of vaginal dryness?

In many cases, vaginal dryness happens when estrogen levels decrease. This occurs naturally as you age or during menopause. Menopause is when your menstrual period ends and you can no longer become pregnant. When estrogen levels decline, the skin and tissues of your vulva and vagina become thinner and less elastic, and your vagina can become dry.

Certain health conditions or treatments for health conditions and can also cause vaginal dryness. Vaginal dryness can result from:

Why is my vagina dry during sex?

Vaginal dryness is usually most apparent during sexual penetration. Without enough vaginal lubrication, the friction (or rubbing) during sexual intercourse can cause pain and discomfort. Take time before sex to make sure you’re fully aroused. Engage in foreplay with your partner and try to relax. Using water-based sexual lubricants can also help. Unfortunately, painful sex can lead to loss of interest in sex or loss of intimacy with your partner. As embarrassing as it may feel, discuss your symptom with your partner so they can help you.

What does a dry vagina feel like?

Vaginal dryness causes discomfort and pain in your vagina, especially during sex. A dry vagina may also cause:

Less moisture in your vagina leads to less moisture in your vulvar area (external genitals). This means you can feel dryness or irritation when putting on your underwear or during normal activities like walking or sitting.

Care and Treatment

How is vaginal dryness diagnosed?

Healthcare providers diagnose vaginal dryness based on your medical history and a physical exam. To find the cause, your provider will ask about your symptoms and any medications you take. They may perform the following tests:

  • Pelvic exam to view the inside of your vagina, which may be thin, dry and red.
  • Blood test to determine if hormone levels or a health condition are causing vaginal dryness.

Your provider may also test a sample of your vaginal discharge to rule out other causes or to check for signs of infection.

How is vaginal dryness treated?

There are many treatments available for vaginal dryness and painful intercourse (dyspareunia) associated with vaginal dryness.

Medications for vaginal dryness

Medications work by either replacing or acting like estrogen in your body. They’re available with a prescription only.

  • Low-dose estrogen cream, ring or tablet: These medications work by replacing estrogen in your body. Creams and tablets are applied directly into your vagina using an applicator. Most are prescribed for daily use until you find relief, then used weekly as needed. Estrogen-containing rings are placed into your vagina for up to three months, then replaced.
  • Ospemifene (Osphena): This medication is called a selective estrogen modulator (SERM) and is taken by mouth. It acts like estrogen in your body and helps treat painful sex associated with vaginal atrophy.
  • Dehydroepiandrosterone (DHEA): This is another medication that acts like estrogen in your body. It’s a vaginal suppository that helps with painful sex in menopausal people.

Talk to your healthcare provider about the risks and benefits of medication containing estrogen or estrogen-like substances. Estrogen may not be safe for people who’ve had breast cancer or who are at high risk of breast cancer.

They work by replenishing your vaginal tissue and making it wet, which can help with pain during sex.   • Vaginal moisturisers: Apply vaginal moisturizers to the inside of your vagina every few days to keep your vaginal lining healthy. Some examples include Astroglide® and   Replens™,
  • Vaginal lubricants: Apply lubricants just before sexual intercourse to ease discomfort caused by sex. Astroglide® and KY® Jelly are examples of water-based vaginal lubricants. There are also silicone-based and oil-based lubricants from Astroglide®.

What foods increase female lubrication?

There isn’t much research about specific foods you should eat to increase vaginal lubrication. Drinking water and staying well-hydrated helps your body retain its moisture.

How do you treat a dry vagina naturally?

Natural oils such as grape seed, olive, vegetable, sunflower or coconut oils may be a safe home remedy for vaginal dryness. Natural oils should be used as an external lubricant before intercourse. However, oil-based lubricants can damage condoms, so if you are of childbearing age, use only water-based lubricants.

Some providers recommend regular sexual stimulation to help encourage your vaginal tissues to become moist. Another thing to try is a longer period of foreplay before intercourse. Vaginal moisture is tied to arousal. Look for ways you and your partner can increase your pleasure before intercourse.

  • Doesn’t get better with over-the-counter treatments.
  • Is accompanied by severe vaginal bleeding.
